The updated 2026–2027 Pfizer-BioNTech COMIRNATY COVID-19 vaccine is now available. This year's vaccine targets the XFG variant, helping the immune system recognize currently circulating strains of COVID-19. The FDA approved the updated formulation on August 27, 2026, for adults 65 and older and younger patients with certain high-risk medical conditions.

How Much Can Vaccination Reduce Emergency Room Visits and Hospitalizations?
Recent research provides encouraging evidence of the benefits of updated COVID-19 vaccines for older adults.
A June 2026 study published in JAMA Network Open examined the effectiveness of the 2025–2026 COVID-19 vaccines among adults across seven U.S. states. Among adults 65 and older without immunocompromising conditions, the study found:
The Protection That Matters for Older Adults
48% - Lower risk of COVID-related emergency room or urgent care visits
53% - Lower risk of COVID-related hospitalization
These national estimates are based on the previous season's vaccines, measured primarily during the first two to three months after vaccination. They are not MPA-specific outcomes or established effectiveness figures for the new 2026–2027 formulation.
Why is this especially important for our patients?
For a healthy younger person, a COVID-19 infection may cause several uncomfortable days. For an 85-year-old patient with heart failure, diabetes, COPD or limited mobility, that same infection may trigger a medical crisis.
Hospitalization can result in muscle loss, falls, confusion, loss of independence and prolonged rehabilitation. Even a relatively short hospital stay may make it difficult for a frail older adult to return to their previous level of functioning.
Preventing the infection's most serious complications may also help preserve independence and quality of life.
Why We Recommend Vaccination Approximately Every Six Months for High-Risk Patients
COVID-19 protection gradually decreases over time, particularly in older adults and people with weakened immune systems.
For the 2026–2027 season, the California Department of Public Health recommends that adults 65 and older receive two doses of the updated vaccine, approximately six months apart. Patients with moderate or severe immunocompromise may need additional doses depending on their vaccination history.

Not every high-risk patient needs exactly the same schedule. We individualize recommendations according to age, medical history and current guidance.
Is the COVID-19 Vaccine Safe for Older Adults?
COVID-19 vaccines have undergone extensive clinical evaluation and ongoing safety monitoring. A September 2026 review published in JAMA examined the accumulated evidence and reported no new safety signals in the studies included in its review.

Most side effects are temporary, including soreness at the injection site, fatigue, headache and muscle aches.
Serious adverse reactions are uncommon. CDC estimates that anaphylaxis, a severe allergic reaction, occurs at approximately five cases per million COVID-19 vaccine doses.
Myocarditis and pericarditis are rare recognized risks of mRNA vaccination, occurring most frequently in adolescent and young adult males, although they can occur in other groups.
MPA evaluates patients' medical histories and any previous serious vaccine reactions before vaccination. Although vaccination does not eliminate the possibility of infection or complications, it provides meaningful protection against severe COVID-19.
